Leptospirosis KAP Survey

Veterinary Professionals of Arizona,

As many of you may already be aware, Arizona has recently seen an increase in cases of leptospirosis in dogs.  State and county public health and animal health partners are very interested in learning more about the current knowledge, attitudes, and practices of Arizona veterinarians in regards to leptospirosis in dogs.  The following survey will provide valuable information that will be used to help guide the design of educational materials, outreach programs, and recommendations for prevention of animal and human infection.  The survey is anonymous and should take about 10-20 minutes to complete.

We appreciate your participation and partnership in promoting health for all two-and four-legged Arizonans!

Thank you,
